At ACCESS, we use the online portal EpicCare Link to support communication and shared electronic medical record information between ACCESS and community partners to better coordinate care.
ACCESSCareLink is an online portal that extends ACCESS' Epic electronic health record to affiliate community partners to enable easy access to shared patient information. This enhances communication between ACCESS and our partners while allowing for easy follow-up and continuation of care.
Access to this platform must be granted directly by an ACCESS team member.
